Community Fundraiser Donation Request Form
Ubergrippen supports local schools and community groups by donating a 1 month family membership that can be raffled or auctioned off in support of your fundraising efforts. If you would like for your organizaiton to be considered for a donation, please complete this form.
WHO WE SUPPORT: Local schools and community groups that serve the an area within 5 miles of out location.

When making a request, please follow these directions closely to ensure your donation is accepted:
When Entering Name:

- LAST NAME: Enter "COMMUNITY ORG" or "SCHOOL"
- First Name: Enter the Name of the Community Group / School
- DOB: Any date is acceptable
Please answer all fields and upload requested documentation.
Not all requests are approved. Approval is determined based on location, community organization mission, impact on local community, and affiliation the requestor has with Ubergrippen.
